Mercy Health Physicians will be conducting sports physicals for Cardinal Mooney students on Thursday evening, July 21 from 6-8 pm at the school in the Junior Hallway immediately below the auditorium. Enter the parking lot through the Erie Street entrance and enter the building through the doors closest to the temporary office in room 109 (on Erie Street to the left of the auditorium, facing the school).
Cost is $20 per student. Checks can be made to MHP or cash is okay as well. Mercy Health Physicians will donate the entirety of the money back to Cardinal Mooney.
In order to have the physicals performed, the student MUST have the first part of the OHSAA form filled out and signed by the parent or guardian. The physicians will not perform the physicals unless it is completely filled out.
